Cor-pulmonale: a rare presentation in a case of middle lobe syndrome

Brock’s syndrome or middle lobe (MLS) is chronic recurrent collapse of right due to causes which may be obstructive non obstructive. The pathogenesis not completely understood. An expert committee the world health organization defined cor pulmonale as hypertrophy ventricle resulting from diseases affecting function and/or structure lungs.” Cor a common heart disease and leading cause disability death. We are reporting this association in 65y old female who presented emergency with acute exacerbation COPD SpO2=64% room air. Chest X-ray HRCT thorax showed features suggesting MLS ECG shows P /right atrial enlargement. BNP too was elevated. Patient resuscitated put on mechanical ventilation after ABG respiratory acidosis. Appropriate treatment bronchodilators antibiotics given.
